In OH a natural parent or sibling can file an authorization form that gives permission for their identify to be released to the adoptee. if the adoption was finalized between January 1, 1964 – September 18, 1996 the form can be completed at any time. For adoptions after September 18, 1996 the natural parent can sign the disclosure statement when relinquishing the child to adoption.

In addition, one natural parent can not authorize the release of identifying information for the other natural parent. In order for identifying information to be released for both natural parents, each parent must complete and file and authorization for release form. In addition, a natural parent can not authorize the release identifying information pertaining to a sibling of the adoptee. The reverse is also true which is that a sibling can not authorize the release of identifying information for the natural parents or another sibling.

A natural parent or sibling may request the release of additional information to the adoptee by providing such information on a separate sheet of paper. This must be signed, dated, and attached to the authorization for release. This additional information can not pertain to the other natural parent or sibling unless the other natural parent or sibling has filed an authorization for release of identifying information.
